There is no canon reason for how she knows henna but, I believe some of her caretakers in India taught her. Whoever they were, I imagine they were a little more gentle towards Lilith than the scientists she would meet later on with her siblings that just viewed them as tools to be discarded at any point. She was trained to be a doctor and was likely the greatest one of her time, possibly of all time considering it is canon she had these skills as a child since in Throne of Roses flashback she is depicted as being a young teen (somewhere around 12-15) holding a medical chart as you can see in my icon. Her military ranking was that of a Lieutenant, bellow Seth, Cain, and Abel. I imagine it has to do with her pacifism. After all, you wouldn't put someone in the military who refuses to kill in a position of power.

As far as Abel goes, I believe it was a gradual thing. He always came to Lilith for healing and comfort. They grew close through these encounters. It's like she told Azula once, 'I am drawn to people like you'. She always goes toward wounded people and tries to save them. It just so happens that Abel was a wounded soul that completed hers.
